Job Responsibilities:
- Provides direct psychosocial intervention to residents and residents' families.
- Assists resident's families in coping with skilled nursing placement, physical illness and disabilities of the resident, and the grieving process.
- Organize family groups to promote communication and education.
- Works with resident and family at time of admission to outline goals of stay and discharge as appropriate.
- Assist residents to determine how they would like to make decisions about their health care.
- Advocate for the resident exploring possible compromises that keep the resident’s best interest in mind.
- Develops and maintains a comprehensive list of services in the community that accept Medicaid.
- Responsible for timely completion of psychosocial assessments, screening instruments, relevant MDS sections and RAPS.
- Primary liaison with psychological and psychiatric service providers.

Job Requirements
MUST HAVE EXPERIENCE IN LONG TERM CARE
- Must be able to communicate effectively.
- Licensed Social Worker preferred.
- A Bachelor’s degree in Human Services field and one year of supervised social work experience in a health care setting preferred.
- Ability to maintain confidentiality.
- Creative problem-solving skills.
- Must meet all federal, state and local health regulations and pass a post-employment offer physical exam including drug screening, criminal background investigation, and reference inquiries.
- Must be able to relate positively and favorably to residents and families and to work cooperatively with other employees.
